
Wellness Program
At Mosaic Health Center, we know that being healthy is more than just keeping your blood pressure in check. It’s also about empowerment and knowledge around health, having stability in your home environment and access to healthy food, and reducing stressors both financial and mental. Because of this, Mosaic Health Center offers a Wellness Program that is available to all our patients. The program works in various ways to ensure the overall wellness of our patients and community:

Wrap Around Services: Our staff work directly with patients understanding their needs outside of the clinic, connecting them to available services and benefits such as rental assistance or food service programs.

Health Navigation: For patients who receive referrals outside of Mosaic Health Center, our team helps them to arrange their appointments, prepares them for their visits, and negotiates on pricing on their behalf.

Health Education: We work to increase knowledge and understanding around the importance and value of primary and preventative care among our patients and community.

Community Outreach: Our staff is active in the community letting them know about the resources that Mosaic Health Center provides and encouraging people to seek primary and preventative care.
